Clinic Terms

Version 2026-10-29-v1. Offered by Peptide Source Group, LLC.

Clinic Terms Version: 2026-10-29-v1 | Date: October 29, 2026 1. Parties and acceptance These Clinic Terms are offered by Peptide Source Group, LLC (PSG) to the clinic legal business identified in its application (Clinic). They take effect when an authorized Clinic representative expressly accepts these Terms and the Business Associate Agreement (BAA), types their full name and title, and consents to an electronic signature. The acceptance record identifies the Clinic, representative, account and server-recorded acceptance date. PSG offers and agrees to perform under these standard terms upon that acceptance. 2. Provider-only sourcing and account responsibilities PSG is a wholesale distributor and marketing support company, not a physician or pharmacy. Access is for verified licensed clinics and providers, not consumer purchasing. PSG does not provide medical advice, prescribe or dispense to patients. Clinic is responsible for accurate legal business and credential information, authorized account use, protecting its credentials, and lawful clinical decisions, prescribing and patient care. Applying or accepting these agreements does not guarantee verification, product availability or order acceptance. Existing verification and referral processes remain applicable. 3. Business Associate Agreement and patient information Clinic agrees to PSG's Business Associate Agreement, version 2026-10-29-v1, available at /baa, incorporated into these Terms by reference. It imposes obligations on both PSG and Clinic, not merely patient consent. For PHI, the BAA controls any conflict with these Terms. Acceptance does not establish that any vendor or platform is ready to process PHI, does not certify HIPAA compliance, and does not remove an independent privacy or readiness hold. Patient information may be submitted only through a PSG-designated enabled workflow and only when its separate safeguards and readiness requirements are satisfied. Do not send patient information in ordinary email, marketing materials, application fields or order notes. PSG's BAA obligations apply to PHI it receives even if received inadvertently. 4. Orders and communications Clinic will use provider-only sourcing and administrative order support lawfully. Product availability, order review, pricing and fulfillment are governed by the applicable ordering workflow; these Terms do not create new pricing, discounts or payment conditions. Administrative account communications may be sent to the Clinic's designated contact. These Terms do not authorize PHI marketing or sale. 5. Records, versions and questions Clinic may print or save these Terms and the BAA and inspect its electronic acceptance in its Clinic Profile agreements section. This version is retained as accepted; future changes require a separately identified version and express acceptance, not retrospective alteration. Questions and written contractual notices may be directed to info@peptidesourcegroup.com without including PHI; PSG and Clinic will arrange a designated secure channel when necessary. Final legal enforceability should be reviewed with counsel. No representation is made that counsel has reviewed these documents. These bounded Terms add no arbitration, indemnity or liability cap and do not waive PSG's legal duties.
